The Spring Festival holiday is the peak period for eye injuries

On New Year's Eve and the first day of the Lunar New Year last year, the emergency department of the Ophthalmology Center of Zhejiang University Second Hospital received five patients injured by fireworks, three of whom had their eyeballs almost blown out .

Doctor Xu Yufeng on duty recalled that a 45-year-old male patient from Lin'an, Hangzhou, was injured by a close-range explosion when he lit fireworks late at night on New Year's Eve. It was already noon on the first day of the new year when he arrived at the emergency room. Because the explosion will produce a huge impact force at the moment, accompanied by high temperature and high pressure, the damage to the eyeball is self-evident.

"The entire wall of his eyeball had irregular cracks of about 7 to 8 mm, and a large amount of normal tissue such as the lens and vitreous body overflowed." After debridement, suture and anti-infection treatment, the patient temporarily preserved the appearance of the eyeball, but ultimately failed to preserve his vision.

During the Spring Festival, the busiest year in previous years, Zhejiang Second Eye Center received nearly 20 cases of eye injuries caused by fireworks.

The peak incidence of eye injuries caused by fireworks and firecrackers is concentrated on New Year's Eve and the first day of the New Year. The custom of welcoming the God of Wealth on the fifth day of the New Year also brings a small peak in eye injuries. Nearly half of the patients are referred from primary hospitals in surrounding areas.

"During the Spring Festival holiday that year, the five ophthalmology outpatient operating rooms were full of patients with eye injuries. The doctors on duty started suturing the patients at 8 a.m. and had not finished until the doctor on duty arrived." Many of the patients are the breadwinners of their families. Faced with such accidents, Director Ma Jian feels deeply regretful.

"Fireworks are not an option for a lively Chinese New Year." Director Ma Jian said that many firecrackers are very powerful nowadays, and even if they fly into the air, they still have a chance to scatter and easily injure people. He hopes that everyone can choose electronic fireworks instead of traditional fireworks to reduce eye injuries.

The severity of injuries from fireworks depends on the location of the explosion. If the eyes are closed for a moment, blunt trauma may occur. Although the eyes may appear fine, it is worth noting that after the external force hits the eyeball, there may be risks of blindness such as iris and ciliary body damage, lens dislocation, vitreous hemorrhage, retinal hemorrhage and detachment . The condition of patients with ruptured eyeballs may be more serious, prone to bleeding, infection, and even loss of eye contents.

"The patient will feel hot tears flowing and will involuntarily wipe or rub his eyes. This is absolutely unacceptable ." Xu Jingwei was the doctor on duty during this year's Spring Festival. He explained that these actions may squeeze the eyeball and cause the contents of the eye to fall out or cause traction leading to further secondary damage such as retinal detachment.

After an eye injury occurs, if conditions permit, clean gauze can be used to gently stick to the surface of the injured eye . If there is a hard, curved protective shell, it can be covered on the outer layer of the gauze and fit the eye socket to minimize secondary injuries such as bumps and collisions during transportation to the hospital to protect the injured eye.

Generally, emergency debridement and suturing focuses on preserving the integrity of the eyeball, timely collecting the leaked contents, and preventing bacterial infection. However, there are still many patients with more serious injuries who need secondary or tertiary surgery to save their vision.

We would like to remind everyone that the number of people setting off fireworks may increase this year, so everyone should remain vigilant.

In addition to injuries caused by fireworks and firecrackers, there are also many workplace injuries caused by the resumption of work and production.
