In the past two weeks, the emergency department of Zhejiang University Children's Hospital has received three children who were injured after accidentally ingesting corrosive agents. Two of them were admitted to the ICU, and one of them was particularly seriously injured. His lips, mouth, pharynx and throat were all corroded, and his condition was once critical.

Wu Xiujing, director of the emergency department, reminds: Many of the cleaning products available at home are corrosive to a certain extent, especially those that look like gummy candies. Parents must keep them properly. If children accidentally ingest them, the consequences will be disastrous.

Sent to the ICU and still in danger

Xuanxuan, 1 year and 4 months old, is very cute. Her mother never dreamed that her healthy and lively daughter would be admitted to the ICU because she ate the pipe cleaner that was always available at home. When her family found her, she was already crying loudly in the bathroom, coughing from time to time, with a little bleeding on her lips. She was holding an opened bag of pipe cleaner in her hand, and there was scattered white powder on the ground. Her family rushed her to the hospital. After emergency treatment in the emergency room, Xuanxuan was sent to the PICU and put on a ventilator. Fortunately, she was sent to the hospital in time and did not eat too much. Xuanxuan was successfully removed from the ventilator three days later and was no longer in danger of death.

Cancan, who is 1 year and 8 months old, walks more and more steadily. He is lively and active and runs to the toilet from time to time, touching here and there curiously. What his family members didn't expect was that he actually found a bag of caustic soda tablets in the toilet cabinet, opened it successfully, poured the caustic soda tablets into his mouth and ate them.

When his family found out, they quickly snatched it away and tried to rinse it with water, but the wound in his mouth became even worse (this action is dangerous, as strong alkali releases heat when in contact with water, which can cause extensive burns to the oral mucosa). His family quickly sent him to the emergency department of Zhejiang University Children's Hospital. The doctor found that his lips, mouth, and throat were severely swollen and his life was in danger. After emergency intubation, he was immediately sent to the PICU. Cancan became the most seriously injured of these children. He has been in the ICU for more than ten days and is still not out of danger.

Becoming a "killer" that harms children's health

Nearly 50,000 children aged 0-14 die from accidental injuries in China every year, an average of more than 130 per day; according to data from the Chinese Center for Disease Control and Prevention, accidental injuries among children in my country account for 26.1% of all child deaths.

The most common corrosive agents accidentally ingested were pipe cleaners in 16 cases, caustic soda and industrial alkali in 6 cases, and laundry detergent beads in 4 cases. Others included hydrofluoric acid, potassium permanganate, sulfuric acid, various cleaning agents such as bleaching powder, dirt cleaners, dust cleaners, car engine oil cleaners, and hair dyes.

According to statistics, the most common cause of accidental ingestion of corrosives is infants and young children aged 1-3 years old. This is because children of this age are very curious and have learned to crawl and walk, but lack safety awareness and necessary judgment, and will put anything they grab into their mouths; the consumption of household cleaning products in modern families is increasing, and various pipe unclogging agents and kitchen and bathroom cleaners are usually liquid, powdered or granular; concentrated laundry or dishwasher detergents, especially disposable small-package products (capsules, gel bags or gel beads) are becoming more and more common. These products are usually small and colorful, and look similar to candy, so children can easily mistake them for candy and eat them.

After ingestion of corrosive agents, corrosive injuries of the lips, mouth, pharynx, and larynx may occur, as well as corrosive esophagitis, corrosive gastritis, esophageal ulcer formation, aspiration pneumonia, etc. In severe cases, esophageal stenosis, esophageal perforation, tracheoesophageal fistula, vocal cord paralysis, aortic arch perforation, mediastinitis, vertebral discitis, gastric bleeding and perforation, intestinal perforation, and even death may occur. The incidence of esophageal stenosis after ingestion of alkaline corrosive agents is 47% to 90%, and the incidence of esophageal stenosis after ingestion of acidic corrosive agents is 6.5% to 50.0%. Literature reports that children may develop digestive tract tumors 15 to 40 years after ingesting corrosive agents. The incidence of digestive tract tumors in children who have had corrosive damage to the digestive tract is 3,000 times higher than that of those who have not ingested corrosive agents.
