We highly suspect that this patient may have star fruit poisoning.

After understanding the cause

Why is carambola so popular?

Carambolas are rich in antioxidants, vitamin C and potassium. With their sweet and sour taste and star-shaped shape, they are an indispensable part of a fruit platter. In my country, they are mainly distributed in Guangdong, Guangxi, Hainan and other places.

Although the taste of carambola is not as pleasing as strawberries and grapes, its low sugar content and ability to stimulate the production of body fluids have attracted much attention. However, we cannot ignore the various cases of carambola poisoning!

Previously, someone summarized the clinical manifestations of 32 patients with carambola poisoning and found that among the various symptoms of carambola poisoning patients, persistent hiccups accounted for 93.75%, vomiting accounted for 68.7%, mental confusion and varying degrees of consciousness disorders accounted for (65.7%); in addition, it also included limb numbness and abnormal sensation (40.6%); hypotension and shock (9.3%), and even death in severe cases.

Why might eating star fruit cause poisoning?

Like spinach, star fruit is a food high in oxalic acid, which is the source of its astringent taste.

Oxalic acid can irritate the gastrointestinal mucosa, causing diarrhea and abdominal pain; it can combine with Ca2+ in the blood to form calcium oxalate crystals that deposit in the renal tubules, causing acute tubular necrosis and interstitial nephritis.

In addition, carambola contains a neurotoxin. Normal people can metabolize and excrete this neurotoxin through the kidneys, but people with abnormal renal function cannot effectively excrete carambola toxin (Caramboxin), which eventually leads to poisoning symptoms.

Symptoms of carambola poisoning

Generally speaking, the symptoms of carambola poisoning are divided into three levels: mild, moderate, and severe:

1. Mild poisoning manifests itself as hiccups (commonly known as burping), nausea, vomiting, abdominal pain, insomnia, etc.

2. Moderate poisoning manifests as agitation, limb numbness, paresthesia, mild disturbance of consciousness, etc.;

3. Severe poisoning is manifested by severe impaired consciousness, coma, epilepsy, hypotension, shock, etc.

If not treated in time, the patient's condition will progress from mild to severe, and the more severe the condition, the higher the mortality rate.

Symptoms of carambola poisoning

Doctors remind that many people like to eat star fruit, and many people make their own star fruit wine.

People with chronic kidney disease, renal insufficiency, renal failure, patients with a history of hemodialysis, and patients undergoing peritoneal dialysis are advised not to eat carambolas or processed byproducts such as carambolas juice, canned carambolas, and carambolas wine. Infants, young children, people who are allergic to carambolas, and patients with hypertension and diabetes should also avoid eating carambolas.

Generally speaking, normal (healthy) people rarely get poisoned when eating carambola and will not experience any adverse reactions. However, they should also avoid eating large amounts of carambola or eating it on an empty stomach. It is recommended to eat no more than one (100 grams) at a time.

For healthy people, it is best not to eat too many carambolas or drink a lot of carambolas juice at one time; do not eat carambolas on an empty stomach, especially when tired or dehydrated. The oxalic acid content in carambolas decreases as the fruit matures, so it is safer to eat carambolas at maturity and remove the ridges and core.
